How a Facelift and RF Microneedling Work
A facelift lifts under the skin, not on it. The surgeon works on the fat and muscle beneath the surface, moves or removes loose skin, and tightens what sits underneath, and the same operation can include the jowls and the neck.
Surgery changes the shape of a face without changing the quality of the skin covering it. Moving tissue does not alter the texture, thinning, or fine lines that came with age.
RF microneedling works in the other direction. Ultra-fine needles enter the layer below the surface and release controlled radiofrequency heat, which tightens the collagen already there and prompts new collagen and elastin underneath. That new structure pulls the skin back in, so it does lift, from the inside rather than by moving anything. As a facelift alternative, RF microneedling changes the skin itself: nothing is cut, moved, or removed, and the skin rebuilds its own support.

Microneedling vs Facelift for Jowls and a Sagging Neck
RF microneedling can firm jowls and tighten the skin along the jawline and cheeks, and it cannot move that tissue back to where it used to sit. Choosing microneedling or facelift surgery for the lower face depends on how far the sagging has gone. A jawline that is softening and has lost its firmness is a collagen problem. A defined fold of loose skin hanging below the jawline is a repositioning problem, and repositioning is surgical work.
We treat jowls, the lines between the eyebrows, smile lines, and general loss of firmness with RF microneedling. A non surgical facelift for jowls works by firming and thickening the skin over the jawline, and the improvement builds across a series of sessions.
The same is true of the cheeks. Flattening and deepening smile lines that come from thinning collagen improve with RF microneedling, because the treatment adds structure back into the layer where those changes start. Cheek fat that has visibly dropped has moved, and moving it back is surgical work.
Advanced jowling does not rule RF microneedling out. Jennifer has seen pronounced jawlines improve with it. What changes when the skin is looser is the number of sessions and the length of the plan.
A mini facelift sits between a full facelift and RF microneedling. It moves a modest amount of skin with a shorter recovery than a full facelift, and the people weighing microneedling vs mini facelift are deciding at the point where the jawline has softened rather than folded. That is the stage where firming the skin can still change how the jawline looks.
A facelift or neck lift moves and removes tissue, so it can lift a sagging neck and a heavy jawline in a way skin tightening on its own cannot. What RF microneedling offers the neck is skin quality: thinning skin, the fine horizontal lines that show up first, and the early loss of firmness that shows up before anything droops. For a neck with true banding or a hanging fold, the texture and firmness will improve and the fold will still be there, so RF microneedling works alongside surgery rather than instead of it.
When you want that much change, or you want it soon, surgery matches the goal more directly.
Jennifer’s insight: I brought RF microneedling into the clinic because I was getting jowls myself and I wanted an option that was not surgery. I started 12 years ago with monthly sessions for the first four months. It worked for me.

Around the eyes the two options barely overlap. An eyelid lift, or blepharoplasty, removes or repositions excess skin, muscle, and fat around the eye, and it leaves crow’s feet, other facial wrinkles, and the dark circles under your eyes unchanged. RF microneedling improves the skin and firmness. Fine lines and thinning skin around the eye are skin-quality concerns, and taking tissue out does not improve either one.
At a shallow, calibrated depth and conservative energy, RF microneedling can treat the skin around the eyes and improve the fine lines there. For microneedling under the eyes, that means the texture can soften and the skin can thicken slightly while the eyelid itself stays as it is. This is precision work, and it depends on a practitioner who can set depth and energy for skin that thin.

Whether that is enough depends on what is bothering you. If it is the texture of the skin, RF microneedling can improve it without the recovery surgery involves. Recovery, Session Time, and Cost
Surgery puts the whole commitment into one procedure and one recovery. RF microneedling spreads it across a series of appointments over months, so the time and the cost accumulate as you go.
A facelift carries the risks of anesthesia and surgery, along with bleeding under the skin, damage to the nerves that control facial movement, wounds that heal poorly, lasting numbness, scarring, and unevenness. Swelling, bruising, skin discolouration, tenderness, and numbness run 10 to 14 days or longer, and the face keeps settling after that.
A full-face RF microneedling session runs about 20 to 30 minutes once the numbing cream has had time to work. You feel mild heat and pressure rather than pain, and most clients return to normal activities the same day.
The first treatments are scheduled closer together. After the initial series, we usually recommend about three sessions a year. Your practitioner adjusts the exact timing to your skin, response, and goals. Results keep building for up to three months after each session as collagen production continues, and how long microneedling results last is what sets your maintenance schedule.

When Surgery Matches the Goal More Directly
Book the surgical consultation when the problem is position rather than firmness. A jawline with a defined fold, cheeks that have visibly dropped, and a neck with loose hanging skin are repositioning problems, and repositioning is surgical work. Everything short of that, firmness, texture, and early sagging, is skin work, and that is what RF microneedling does.
How far RF microneedling goes depends on how many sessions you are willing to do. We have not had to tell a client their skin was beyond what we could treat, and no one has told us the discomfort was not worth the result.
